Are there any steps that must be taken prior to filing a grievance?
No steps must be completed prior to filing a grievance. Grievance information can be found in the State agency brochure.
What information and documents are required in order to file the grievance?
You will need to give your name and the reasons for the complaint. Specific documents not required.
How and where to file the grievance?
To file a grievance, visit your local Children’s Division office and ask for a Service Delivery Grievance Form (CS-131). Grievance must be filed within 30 days of service. For information on local agencies contact choose your county from the drop-down menu.
