What information and documents are required in order to file the grievance?
Required information includes:
- Your name and address
- Name and address where the alleged offense took place
- A description, including the date, of the offense
- A desired remedy
How and where to file the grievance?
To file a complaint, complete the DHS 0170 form
You may email the completed form to: gao.info@odhs.oregon.gov
or drop it at your local ODHS office.
Timeline for submitting complaint:
The complaint should be submitted as soon as possible but not later than sixty (60) days after the offense. Contact the Governor's Advocacy Office for information and assistance by phone, 503-945-6904/800-442-5238 or by email, gao.info@odhs.oregon.gov.
What to expect after filing and how to track the progress of your grievance?
The designated supervisory employee will contact the complainant to schedule an in-peron or telephone meeting. If complaint is not resolved, it will be forwarded first to the Program Manager, then, to the Service Delivery Area or Regional Program Manager and to the Governor's Advocacy Office (GAO) for follow-up action.
